Custom Forms, Made Easy
Lobbie handles all form building for you. Simply provide your existing paperwork, and our team will digitize and configure it within your Lobbie environment. You can also choose from a library of pre-built templates if you don’t have existing forms.
Sending Forms to Patients
With just a few clicks, you can send forms to one or more patients:
- Click Send Forms in the top right.
- Choose to send via SMS, email, or QR code.
- Enter patient details (you can add multiple patients at once).
- Choose a due date for form completion.
- Select the correct form packet or use filters to find specific forms.
- Confirm and send.
Patients receive a branded message with a link to their forms, no login or account setup required. They can complete forms on any device, and 70% of patients complete them on mobile.
Form Reminders and Notifications
- Automated reminders are sent if forms aren’t completed.
- Reminders stop once forms are submitted.
- Staff can opt in to receive an email notification when a patient completes their forms.
