Overview
In Lobbie, Intake Forms, Clinical Notes, and Form Packets are fully customizable to fit your workflows. This article covers how to:
- Request edits to existing forms
- Automatically attach forms to appointment types
- Create and manage reusable form bundles (template groups) for sending forms without an appointment
Creating or Editing Intake Forms and Clinical Notes
All Intake Forms (patient-facing) and Clinical Notes (staff-only) are fully customizable.
To request edits, please email support@lobbie.com and include:
- The name of the Intake Form or Clinical Note
- A list of the fields to be added, removed, or edited, including exact label text and any instructions
- Any preset values/macros you’d like include:

Creating Form Packets for Appointment Types
You can pre-define which forms are attached to specific appointment types so they’re automatically selected when a patient is scheduled.
How to Create or Edit Form Packets for Appointments
- Go to Settings > Appointment Types
- Click Edit next to the appointment type you want to configure
- Under the Forms field:
- Select all Intake Forms you want to send to the patient
- Optionally select Clinical Notes (these will be attached internally and not sent to the patient)
- Click Save Appointment Type
Once saved, the forms will automatically be selected whenever this appointment type is used.
Creating Form Packets for Manual Form Sending (No Appointment)
To send forms manually without scheduling an appointment, you can create Form Template Groups (form bundles).
How to Create a Form Template Group:
- Go to Settings > Forms
- Use the checkboxes to select the form templates you want to include
- Click the “Create Template Group” button in the top-right
- In the popup:
- Give the group a name
- Select a location (required)
- Optionally assign a practitioner (forms sent using this group will assign the patient to the selected provider)
- Click Save to create the template group
Editing a Form Template Group
- Scroll to the Form Template Groups section in Settings > Forms
- Click the Edit link on the group you want to update
- You can edit:
- The form templates included in the group
- The name, location, or practitioner
Note: Updates apply to future use of the template group. Previously assigned forms will not be changed retroactively.
