Overview
In Lobbie, you can choose which forms are sent to patients based on the type of appointment they book. This allows you to tailor forms to the specific services you offer, such as intake forms for initial visits or consent forms for lab work.
How to Attach a Form to an Appointment Type
- Go to the Settings section in Lobbie
- Click on Appointment Types
- Select the appointment type you want to edit (For example, “Initial Hormone Consult”)
- Click Edit
- Under the Forms section, you’ll see the list of templates currently set to go out
- Click Add to include additional forms (For example, you might add a lab form or medical history form)
- Click Save Appointment Type to confirm your changes
Example
If you want a lab form to go out for all Initial Hormone Consults:
- Go to Appointment Types
- Select Initial Hormone Consult
- Add the Labs form
- Click Save
The selected form will now automatically be sent to the patient when that appointment type is scheduled.
